SA-Exim lets you run SpamAssassin at SMTP time so that you can reject mail early without being a joe job source.
It also allows for teergrubing, or adaptive greylisting depending on the SA score (i.e. don't greylist ham, reject spam right away, and greylist what's in the middle)
